By agreeing below, I take full responsibility of access card issued to me. I will not allow any other person use this card other than myself under any circumstances. I agree if this card is lost or stolen, I will immediately report it to UCR Police Department, UCR Arts Administration staff and the department designee. I agree this card is property of the University of California and must be forfeited at anytime. I agree that failure to adhere to these rules will result in confiscation of card and denied access to above facilities.
